MCPS New Student Enrolment Form for 2021

Welcome
This form is for Enrolment in the Mt Colah Public School (MCPS) Band Program. All the costs involved with being in the band (such as Band Fees, Instrument Hire, Lesson Costs & Band Camp) are listed in the form below – please scroll down to view. Enrolment Information is used by the MCPS Music Committee and Key Music Australia to process your enrolment.

Instrument Preference

Lesson Type
For new beginners, we recommend a shared lesson (2 students in a lesson) for the first year of lessons. After one year of shared lessons, students will need to progress to a private lesson. All music lessons in 2021 will be held on-site at school, during school hours.

**PLEASE NOTE shared lessons are not available for percussion students**

**SHARED lessons are not always suitable for every student. Children need to be able to concentrate, stay focussed and work in a group environment for 30mins. Students who are found to not be suitable for shared lessons may need to be moved to private lessons at the discretion of Key Music Australia staff.**
Band Fees
The Band Fee in 2020 for the Waratah Band was $70 per term. We expect the 2021 fee to be similar. A discount is available for the full year paid in advance. This fee includes the cost of conductors at rehearsals and performances. This fee will be invoiced by the MCPS Music Committee (not Key Music Australia).
Instrument Hire
The MCPS Music Committee has several instruments available to hire for members of the Waratah (Training) Band. You will be advised by the end of Term 4 if your application to hire an instrument has been successful. The hire fee of $60* per term includes annual servicing and cleaning of the instrument. Any supplementary services or repairs during the year are the responsibility of the hirer. Please note that instruments hired from the MCPS Music Committee are invoiced directly by the MCPS Music Committee (not Key Music Australia).

*Please note this is the 2020 price. We expect the 2021 price to be similar.
Band Camp
Band Camp is held each year. The 2021 Band Camp will be held in Term 3 on Sat 24th & Sun 25th July. The cost of Band Camp in 2020 for Waratah Band members was $225 per student with the fee inclusive of all meals, tutorials, band/ensemble sessions & activities. This fee is invoiced directly by the MCPS Music Committee (not Key Music Australia). We expect the 2021 fee to be similar.
Parent Involvement
Key Music Australia works in partnership with the P&C Band Committee and School staff to run the MCPS Band Program. Parent volunteers are essential to the success of the program. Families are needed to assist with the running of the band program. There are a number of small jobs that need to be done throughout the year. Some jobs need to be done on a weekly basis, others are only a few times a year. If every family does a small amount throughout the year, the jobs can be shared around easily. Please consider helping out so your children can benefit the most from their time in the school band.
**Ticking a box here is not committing you to a specific job. It will just allow the band committee to make contact with families who may be to able to assist at some point during the year**

The below information will greatly assist us to best meet your child's needs in regards to music tuition. This information is used confidentially and will be provided to your child's conductor/tutor as needed.
Photo & Video Consent
Key Music Australia will at times, take photos or videos of students to assist with students education. This images and videos will be used by Key Music Australia Staff for professional development. Photos and Videos may also be posted on the Key Music Australia websites (www.keymusicaustralia.com.au & www.myschoolband.com.au).
Consent & Agreement
By submitting this form, I agree to the Key Music Australia Terms & Conditions (CLICK HERE TO VIEW ON OUR WEBSITE) I understand that my invoice will be sent to me via email each term and I agree to pay this invoice by the due date listed on the invoice. I understand that if my invoice is not paid by the due date, my child will not be able to attend music lessons or band rehearsals and the outstanding invoice amount will be referred to our debt collection agency as per our Terms & Conditions. All information collected by Key Music Australia will remain private and confidential and will only be used by staff and music committee members of Mt Colah Public School and the staff and tutors of Key Music Australia and will not be disclosed to anyone.
